I acknowledge that risks may include\, but are not limited to\, skin irritation\, allergic reactions\, itching\, redness\, swelling\, discoloration\, or other unforeseen skin sensitivities. I voluntarily assume all risks\, known and unknown\, associated with receiving a henna tattoo. \nMedical Acknowledgment\nI confirm that I (or the minor participant) do not have any known allergies\, skin conditions\, or medical issues that would make participation unsafe\, or I have chosen to proceed despite such conditions.
